The Church in Columbia

RELEASE OF LIABILITY
I hereby acknowledge and certify that I, being of lawful age, and in consideration of being permitted to gain access to and participate in any and all Bible Camp activities, events or gatherings for the year of 2023 or at any future date and time while attending the Events ("Events" shall mean the 2023 Bible Camp hosted and coordinated by the Church in Columbia), release and forever discharge the Activity Provider ("Activity Provider" shall mean and include: the Church in Columbia including its officers and members; Carolina Christian Students including its officers, members, volunteers, and staff) from any and all claims, liabilities, obligations, promises, actions, debts, accounts, bonds, contracts, agreements, disputes, damages, demands or causes of action of any nature and kind, known or unknown, which I may have or in the future may have against the Activity Provider for or by reason of any and all injury to my person or property, including but not limited to death resulting from injury ("Damages"), that I might sustain as a consequence of my participating in any and all activities, events or gatherings on or around the during the Events even notwithstanding that such Damages may have been caused solely or partly by the Activity Provider's negligence.

 

MEDICAL RELEASE

I further hereby acknowledge and certify that in the event of an emergency or non-emergency, I give my consent that the Activity Provider may administer emergency or non-emergency medical aid for any accident or illness as deemed necessary including but not limited to receiving first aid, trauma care, and/or to be taken to a medical facility or act in my stead in approving necessary medical care if I am rendered unable to make such decisions and, if applicable, no guardian is present ("Administered Aid").  I release Activity Provider from any and all responsibility, liability and cost for any and all Administered Aid and I assume the responsibility of any accident, injury, illness or liability and any and all subsequent medical bills and expenses accrued while participating in the Events and due to any Administered Aid at the locations of the Events or at and while in transit to a medical facility. 

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
